Necessary Registration for Maximum Assurance

The mission of MyHealthDirective.com is to promote communication related to living wills/advance healthcare directives (AHD), and assist families in planning for their future. We also provide a secure online registry to store these documents, a NECESSITY to ensure healthcare professionals can access your instructions.

Because the laws regarding AHDs vary from state to state, this site provides a "state selection" prompt prior to the retrieval of state related information.

To become a member, purchase Five Wishes online now in our Five Wishes section above, and register it with us. If you already have an AHD, then simply download the Registration Agreement by clicking the "Download Form" button to the upper left, and follow the instructions. If you become ill and are not able to communicate your healthcare wishes, your physician will be able to go to MyHealthDirective.com and print a copy of your AD. Even if you are still capable but want someone else to make your healthcare decisions for you, you can designate that person in your AD so that this information is available to your physician.

You may also let family members and/or your chosen proxy view your AHD by providing them with your password.

  • A useful clearinghouse of information and resources regarding the development of employee benefit programs related to end-of-life care
  • A vehicle to obtain materials for the Five Wishes Program
