Confidential Storage and Retrieval of Advance Healthcare Directives

The MyHealthDirective.com system registers and stores Five Wishes, and other Advance Healthcare Directives (AHD), for individuals residing in the United States of America.

As a member, you will have the security of knowing that your AHD will be available to your treating physician and hospital, should you be unable to express your wishes or locate your AHD. Here's how to become a member:

  • Purchase your Five Wishes plus storage AHD from one of our Sponsors. If you already have an AHD, click on the "Download Form" button to the upper left, and download a Registration Agreement.
  • Submit your orginal Five Wishes AHD (witnessed or notarized) to MyHealthDirective.com. The Registration Agreement is inserted in the center of the document. If you are registering an existing AHD, submit the original with the downloaded Registration Agreement and payment to MyHealthDirective.com.

Once you have submitted your AHD to us, we will:

  • Scan your AHD into the MyHealthDirective.com database, exactly as submitted.
  • Input your contact information.
  • File and store your original AHD in a secure location.
  • Send you a confirmation letter and wallet card that provides you with a User ID and password. Your User ID and password will allow you and your healthcare professionals to enter the secured member section of MyHealthDirective.com.

As member, you can view, print, or download a copy of your AHD from MyHealthDirective.com. You may also change or revoke your AHD at any time.

Physicians and hospitals may also view and print their patients' AHD in time of need, by accessing the information contained on your wallet card.

Please note: There may be situations beyond our control in which MyHealthDirective.com will not be able to provide a copy of your AHD to your hospital or treating physician. Thus, it is important to provide copies of your AHD to your primary care physician and your personal representative, or to make them aware that you have an AHD and to keep a copy in a place that is accessible in the case of an emergency. You may download as many copies of your AHD as you need from this Web site.
