How to Change Your AHD

Replacing and Revoking Your Advance Healthcare Directive

To change your AHD, you must submit a new one to replace the one currently on file with us. You will not be able to change your AHD on-line.

To submit a new AHD, you must purchase a new Five Wishes AHD and submit it with a brief letter indicating your current membership number to MyHealthDirective.com. To submit a non-Five Wishes AHD, you must submit it with a brief letter indicating your current membership number to MyHealthDirective.com.

You may revoke your AHD at any time by submitting your request in writing to:
